and you read them "upwards" ...

so c) caused b) which led to a) which was the actual cause of death

Thank you Antony - I was never quite sure how causes "1a", "1b" and "2" (say) hung together. Presumably any "2" is just an "also..."

Any condition listed as 2 or II is something that may have contributed to the death but not directly relating to (or the cause of) the actual cause of death.
