Who would have made the Green entry on the 1921 Census original entry and what does it read. B O Father Dead?

It would be made by the checking clerks when the returns were being collated, I would also add it looks as if it was being amended to both dead but changed to father dead, so there was obviously some confusion whether one parent or both parents were deceased.

From the Birth Certificate of the girl in question her mother died upon giving birth but her father named on the BC is on the Census and filled it in for the head of the house.
This is a complicated situation.
Man A marries Girl B and a year later mother dies giving birth to Girl C
Man A leaves for Australia 4 months later leaving girl C with Neighbour
1915 Man A joins Australian Army and sails to Galipoli for WW! but too late as battle was lost and goes onto France naming Girl C as dependant and Neighbour as Guardian
Man A returns to England after war and Marries (1920) daughter of Neighbour he left child with
On 1920 Census Girl C now has surname of Neighbour despite Birth cert stating (Fathers Man A) Name
Man A is correctly shown as Son in Law
It is difficult to understand why Girl C has had her surname changed

Not really.
To save the girl having to explain to everyone why her surname was different from her ‘mother’, she used her mothers surname. She obviously felt closer to her ‘mother’ than her father, so kept her surname rather than change back to her real surname.
Children especialy, can be quite cruel to children who are different, e.g. having a different surname from their mother.
